A 1.5-inch segment of roughly 200 hair strands (comparable to a #2 pencil's thickness) near the scalp provides 100mg of hair, which is the optimal amount for screening and confirmation. For EtG, additional tests, and/or panels beyond ten, 150mg of the hair sample is advised. We suggest weighing the sample on a jeweler’s scale. If scalp hair is unavailable, an equivalent quantity of body hair may be used. When mentioning head hair, we specifically mean scalp hair. Body hair includes all other types (facial, axillary, etc.).

Process Overview

The laboratory procedure for processing a drug test outcome encompasses four main steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the preliminary processing of a sample within a laboratory’s protocol. This step involves ensuring the sample was correctly sealed and delivered, assigning a random LAN (Laboratory Accessioning Number), and completing any supplementary data entry not captured by an electronic chain of custody system.

Screening permits an initial swift assessment for substances of abuse. While it is an economical strategy to dismiss drug usage in the majority of specimens, a positive screen must be verified to be legally valid. Samples indicated as presumptively positive during Screening require additional confirmation.

Confirmed positive screens undergo validation via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptively positive samples are cleansed before validation as necessary. The entire laboratory sequence from Accessioning to Confirmation undergoes review under both the CAP (College of American Pathologists) Hair designation and accreditation in line with the ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
