A 1.5-inch portion consisting of approximately 200 hair strands (comparable to the girth of a #2 pencil) nearest to the scalp results in 100mg of hair, which is the optimum amount for screening and confirmation. For EtG, additional tests, and/or testing beyond 10 panels, a 150mg sample is advised. It is suggested to weigh the sample on a jeweler’s scale. If scalp hair is not available, a similar amount of body hair can be utilized. When we mention head hair, we mean scalp hair exclusively. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result encompasses four primary phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial integration of a sample into a lab’s system. This phase covers confirming the sample was properly sealed and dispatched, assigning a random LAN (Laboratory Accessioning Number), and completing any missing data not included by an electronic chain of custody system.

Screening offers a preliminary, efficient check for substance abuse. Although Screening is a cost-effective means to dismiss drug use for most samples, any positive result necessitates confirmation for court acceptance. Samples suspected positive in Screening must undergo subsequent confirmation.

If Screening suggests a positive result, additional hair is drawn from the initial sample for Extraction. At this stage, drugs are isolated from hair at substantially lower concentrations compared to other methods (such as urine or oral fluid), which is why hair drug testing is the most intricate methodology available.

Positive screening results are confirmed via GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positive results undergo washing prior to confirmation if needed. The entire process from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.
