A sample measuring 1.5 inches and comprising roughly 200 hair strands (around the thickness of a #2 pencil) near the scalp provides 100mg, an ideal quantity for both screening and confirmation tests. For EtG, additional analyses, and/or evaluations exceeding 10 panels, a specimen of 150mg is advised. We suggest using a jeweler’s scale for measuring the specimen. If scalp hair is not attainable, a comparable quantity of body hair might be gathered. References to head hair specifically denote scalp hair. Body hair includes all other types of hair (facial, underarm, etc.).

Process Overview

The laboratory procedure for processing drug test results comprises four principal stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the sample's initial integration into a lab's system, confirming it was properly sealed and dispatched, assigning a unique LAN (Laboratory Accessioning Number), and completing any extra data recording not available via an electronic chain of custody system.

Screening is a preliminary assessment for substance misuse. While it is a cost-efficient method to eliminate drug presence in most samples, any positive result must be confirmed to be acceptable in legal settings. Samples with a presumptive positive result from Screening require follow-up confirmation.

If a sample tests positive in Screening, additional hair is taken from the original sample for the Extraction phase. Here, drugs are separated from the hair at significantly lower concentrations compared to other techniques (such as urine or saliva testing), making hair drug analysis the most intricate technique to execute.

Confirmation of a positive Screening outcome is performed using GC/MS, GC/MS/MS, or LC/MS/MS technologies. All presumptively positive samples are cleansed before confirmation if necessary. The laboratory workflow from Accessioning through Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and compliance with ISO / IEC 17025 criteria.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
