What it is, How it works

When sourced near the scalp, hair samples can present up to a roughly 3-month detection period for substances. Hair is straightforward to collect, challenging to tamper with, and simple to transport.

A sample of 1.5 inches, approximately 200 strands of hair (comparable to the size of a #2 pencil) nearest to the scalp, will yield 100mg of hair, which is the ideal quantity for analysis and confirmation. For EtG, additional tests, and/or assessments beyond 10 panels, a sample of 150mg is advised. We advocate using a jeweler’s scale to measure the sample. If scalp hair is unobtainable, an equivalent measure of body hair can be used. When discussing head hair, we are exclusively referring to scalp hair. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The four major stages in processing a drug test result in the lab include Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the initial setup of a sample into a laboratory's framework. This process comprises verifying the proper sealing and shipping of the sample, attaching a random LAN (Laboratory Accessioning Number), and completing any data input not provided by an electronic chain of custody system.

Screening is an initial quick assessment for drugs of abuse. While Screening is a cost-efficient means to dismiss drug use in most samples, a positive screen mandates confirmation to be legally valid. Any samples that test potentially positive in Screening necessitate a follow-up confirmation.

If a sample shows as potentially positive in the Screening phase, additional hair is extracted from the main specimen and prepped for Extraction. At this juncture, drugs are isolated from hair at a much-reduced concentration compared to other approaches (e.g., urine or oral fluid), which explains why hair drug screening is the most challenging methodology to execute.

Confirmation of any positive screening result is carried out using GC/MS, GC/MS/MS, or LC/MS/MS. All potential positive samples undergo washing prior to confirmation as needed. The complete lab process from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
