A sample measuring 1.5 inches comprising around 200 hairs (akin to a #2 pencil’s thickness) nearest to the scalp yields 100mg, which is ideal for both screening and confirmation purposes. For EtG, add-ons, and/or tests exceeding 10 panels, a specimen of 150mg is advisable. We suggest measuring the specimen using a jeweler’s scale. Should scalp hair be unavailable, a similar quantity of body hair may be harvested. Head hair specifically pertains to scalp hair, whereas body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The laboratory handling of a drug test outcome involves four chief phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial stage where a sample is input into a lab’s system. This includes confirming that the sample was duly sealed and dispatched, issuing a random LAN (Laboratory Accessioning Number), and finalizing any additional data entry omitted by an e-chain of custody system.

Screening serves as an initial cursory assessment for drugs of abuse. While Screening offers a cost-efficient means of excluding drug usage across most samples, a positive screen mandates confirmation for court acceptability. Samples that presumptively test positive during Screening require further confirmation.

Should a sample show presumptive positivity in the Screening phase, additional hair is extracted from the initial specimen and readied for Extraction. In Extraction, drugs are drawn from hair at much lower concentrations than in other methods (e.g., urine or oral fluid), marking hair drug screening as a challenging technique.

Confirmation for any positive screening outcome employs GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive specimens undergo washing before confirmation if necessary. The entire lab process from Accessioning to Confirmation undergoes scrutiny under the CAP (College of American Pathologists) Hair designation and aligns with ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
