What it is, How it works

The hair captures a prolonged record of alcohol and drug consumption by embedding biomarkers within the fibers of the growing hair strand. When taken near the scalp, hair can provide a detection period of up to roughly three months for alcohol and drugs. The collection process is straightforward, somewhat resistant to tampering, and easy to transport.

A hair sample measuring 1.5 inches with about 200 strands (similar to a #2 pencil) nearest to the scalp yields 100mg of hair, which is the optimal amount for screening and confirmation. For EtG, additional analyses, and/or tests exceeding ten panels, 150mg of the sample is advised. We suggest using a jeweler's scale to measure the sample. If scalp hair cannot be obtained, a comparable amount of body hair may be used. When mentioning head hair, we specifically mean scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The main procedures in the laboratory handling of a drug testing result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial phase of processing a sample within a laboratory's framework. This step includes verifying the sample's sealing and shipping integrity, assigning a random LAN (Laboratory Accessioning Number), and finalizing any extra data entry not covered by an electronic chain of custody system.

Screening entails a preliminary quick assessment for drugs of abuse. Though Screening is cost-effective for ruling out drug use in most samples, a positive screen must be validated to be legal in court. Samples that test presumptively positive during Screening require a secondary confirmation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field.
