When trimmed close to the scalp, hair can reveal a detection period of up to nearly 3 months for alcohol and other drugs. Hair collection is straightforward, relatively challenging to tamper with, and convenient to transport.

A 1.5-inch segment of roughly 200 hairs (comparable to the diameter of a #2 pencil) near the scalp will yield 100mg of hair, which is the optimal amount for screening and validation. For EtG, supplemental tests, and/or analyses exceeding 10 panels, 150mg of the sample is suggested. It is advisable to use a jeweler’s scale for weighing the specimen. If scalp hair isn't available, an equivalent quantity of body hair can be taken. When we mention head hair, we specifically mean scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The key phases in the lab processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the preliminary integration of a sample into a lab’s system. This involves verifying proper sealing and shipping of the sample, assigning a random LAN (Laboratory Accessioning Number), and completing any extra data entry not covered by an electronic chain of custody system.

Screening entails a preliminary rapid assessment for drugs of abuse. Although Screening is an economical approach to negate drug usage for most samples, a positive screening outcome must be verified to be legally acceptable. Samples showing a presumptive positive in Screening need a subsequent confirmation.

If a sample shows presumptive positivity in the Screening phase, additional hair is extracted from the original specimen and prepped for Extraction. In this phase, drugs are isolated from hair at considerably lower concentration compared to other techniques (e.g., urine or oral fluid), explaining why hair drug testing is notably complex.

Confirmation of any positive screening results is carried out using GC/MS, GC/MS/MS, or LC/MS/MS. All samples showing presumptive positivity undergo washing before confirmation if needed. The complete lab process from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
