If scalp hair is not accessible, a similar quantity of body hair is permissible. "Head hair" solely refers to scalp hair. "Body hair" encompasses all other hair types (facial, axillary, etc.).

Process Overview

There are four critical phases in laboratory processing of a drug test result: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the preliminary assimilation of a sample into a lab's system. This step includes ensuring the sample was appropriately sealed and sent, assigning a unique LAN (Laboratory Accessioning Number), and completing any extra data entry not covered by an electronic chain of custody system.

Screening provides an initial rapid drug test. Although Screening is an economical method to exclude drug use in most samples, a positive result requires a confirmation to be legally valid. Presumptively positive samples in Screening demand further confirmation.

When a sample appears positive during Screening, additional hair is extracted from the original sample for Extraction. In this phase, drugs are extracted at a significantly lower concentration than other methods (such as urine or oral fluid), making hair drug screening the most complex technique to execute.

Confirmation of any positive screening outcome utilizes GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples with presumptive positives are cleansed before confirmation as applicable. The entire laboratory procedure from Accessioning to Confirmation is overseen under the CAP (College of American Pathologists) Hair certification and accredited to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
