When sourced near the scalp, hair can offer a detection span of up to roughly 3 months for alcohol and drugs. The collection of hair is straightforward, less prone to tampering, and simple to transport.

An optimal sample consists of a 1.5-inch length of about 200 hair strands (approximately the size of a #2 pencil) closest to the scalp, yielding 100mg of hair, perfect for screening and validation. For EtG, supplementary tests, and/or panels above 10, a specimen of 150mg is suggested. Use a jeweler’s scale to measure the specimen's weight. If scalp hair is unavailable, the same quantity of body hair may be collected. "Head hair" indicates only scalp hair, while "body hair" encompasses all other types (e.g., facial, axillary).

Process Overview

The laboratory processing of a drug test result entails four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning covers the preliminary incorporation of a sample into a lab's system. This phase includes ensuring the sample was correctly sealed and dispatched, assigning a unique LAN (Laboratory Accessioning Number), and finalizing any extra data input not available through an electronic chain of custody system.

Screening offers a preliminary rapid assessment for drug abuse. While Screening is a cost-efficient method to dismiss drug usage in most samples, a positive screen needs affirmation for court acceptance. Samples that are initially positive in Screening necessitate a further confirmation.

If a sample tests presumptively positive during Screening, additional hair is extracted from the original sample and prepped for Extraction. During this phase, drugs are extracted from hair at much lower concentrations than other techniques (e.g., urine or oral fluid), which is why hair drug screening is among the most intricate methodologies.

Validation of any positive screen result is executed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positives are cleansed before validation as required. The entire laboratory procedure, from Accessioning to Confirmation, is examined under CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 standards accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
