Process Overview

The process within the lab for generating a drug test result consists of four primary steps: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, the sample is initially processed into the lab’s systems. This includes checking that it was sealed and dispatched appropriately, assigning a LAN (Laboratory Accessioning Number) randomly, and finishing any additional data input not provided by electronic chain of custody systems.

Screening involves a preliminary rapid examination for substance abuse. Though Screening is an economical method for excluding drug use in most samples, a confirmed positive result is needed for legal admissibility. Any samples hinting positivity in Screening must undergo a further confirmation.

If a sample shows potential positivity during Screening, additional hair is extracted from the original specimen for the Extraction phase. In this stage, substances are isolated from hair at lower concentrations than those typical in other methods such as urine or oral fluid testing, making hair drug screening the most intricate to perform.

Confirmation of any initial positive result is accomplished using GC/MS, GC/MS/MS, or LC/MS/MS methods. All samples with presumptive positives are washed as necessary before confirmation. The entire lab process from Accessioning to Confirmation adheres to both CAP (College of American Pathologists) standards for Hair and ISO / IEC 17025 standards accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
