What it is, How it works

When cut close to the scalp, hair offers a detection window of around three months for alcohol and other drugs. Hair collection is straightforward, relatively tamper-proof, and easy to transport.

A 1.5-inch segment of approximately 200 hair strands (comparable to the width of a #2 pencil) nearest to the scalp yields 100mg of hair, the optimal sample amount for both screening and verification. For EtG, supplementary tests, and/or tests exceeding 10 panels, 150mg is advised. Using a jeweler's scale for weighing the sample is recommended. If scalp hair cannot be obtained, an equivalent amount of body hair can be used. "Head hair" refers exclusively to scalp hair, while "body hair" encompasses other types (such as facial or axillary hair).

Process Overview

The laboratory processing of a drug test outcome comprises four key stages: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, the sample undergoes preliminary processing into the laboratory system. This involves verifying the sample's proper sealing and shipping, assigning a random LAN (Laboratory Accessioning Number), and completing any supplementary data input not included in an electronic chain of custody system.

Screening entails an initial rapid assessment for drugs of abuse. While Screening is cost-effective for ruling out drug usage in most samples, a positive result from Screening requires confirmation to be valid in court. Any samples preliminarily positive in Screening necessitate additional confirmation.

If a sample tests presumptively positive during Screening, additional hair is taken from the original sample and prepared for Extraction. This phase involves extracting drugs from hair at significantly lower concentrations compared to other techniques (e.g., urine or oral fluid), making hair drug screening one of the most challenging methodologies.

Confirmation of any positive Screening result is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are washed before Confirmation, as needed. The entire laboratory process, from Accessioning to Confirmation, is reviewed under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
