For EtG, add-ons, and/or tests exceeding 10 panels, a 150mg sample is suggested. It is advisable to weigh the hair sample using a jeweler’s scale. If scalp hair is unobtainable, a corresponding amount of body hair can be used. When mentioning head hair, it specifically refers to scalp hair. Body hair includes all other hair types (facial, axillary, etc.).

Process Overview

The laboratory drug testing procedure involves four critical steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial stage where a sample is processed within a lab's system. This involves confirming the sample's proper sealing and shipment, assigning a random LAN (Laboratory Accessioning Number), and finalizing any further data entry that an electronic chain of custody system might not have supplied.

Screening is a preliminary rapid assessment for substances of abuse. Screening is a cost-efficient method for eliminating drug use in most samples; however, to be legally acceptable, a positive screen must be verified. Presumptively positive samples in Screening require a follow-up confirmation.

If a sample tests presumptive positive during Screening, additional hair is extracted from the original sample and prepared for Extraction. In this phase, drugs are isolated from hair at much lower concentrations than other methods (e.g., urine or oral fluid), making hair drug screening the most complex methodology.

Positive screening results undergo Confirmation using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptively positive samples are cleansed before confirmation if necessary. The entire laboratory process from Accessioning to Confirmation is scrutinized under the CAP (College of American Pathologists) Hair designation and adheres to ISO / IEC 17025 standards.
