Hair is easy to collect, somewhat resistant to tampering, and simple to transport.

A 1.5-inch segment of roughly 200 strands of hair (about the diameter of a #2 pencil) closest to the scalp will yield 100mg of hair, which is the preferred sample size for screening and confirming. For EtG, additional tests, and/or screenings beyond 10 panels, 150mg of the sample is advised. It's suggested to measure the sample using a jeweler’s scale. Should scalp hair be unavailable, a comparable quantity of body hair can be used. References to head hair pertain exclusively to scalp hair, whereas body hair refers to any other hair types (facial, axillary, etc.).

Overview of the Process

The procedure for laboratory processing of a drug test result comprises four key stages: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, the sample undergoes initial processing into a lab's system. This stage involves ensuring the sample was properly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and finalizing any outstanding data entry not covered by an electronic chain of custody system.

Screening involves a preliminary rapid assessment for drugs of misuse. While Screening is a cost-efficient means to eliminate drug usage in most samples, a positive screen must be verified to be accepted legally. Any samples indicating a presumptive positive in Screening necessitate secondary confirmation.

When a sample tests presumptively positive in the Screening phase, additional hair is extracted from the initial sample and prepared for Extraction. Here, drugs are withdrawn from hair at significantly lesser concentrations than in other methods (such as urine or oral fluid), which is why hair drug testing is the most intricate method to administer.

Confirmation of any positive screening result is carried out using GC/MS, GC/MS/MS or LC/MS/MS. All presumptive positive samples undergo washing prior to confirmation if needed. The comprehensive laboratory procedure from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair classification and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
