Hair captures a long-term record of alcohol and other drugs by trapping biomarkers in the strands of the growing hair. When gathered near the scalp, hair can offer around a 3-month detection window for alcohol and other drugs. Hair is easy to collect, relatively hard to tamper with, and simple to transport.

A sample measuring 1.5 inches with approximately 200 strands of hair (approximately the thickness of a #2 pencil) close to the scalp supplies 100mg of hair, which is the preferred sample for analysis and verification. For EtG, additional tests, and/or examinations exceeding 10 panels, 150mg of the specimen is preferred. It's advised to weigh the specimen using a jewelry scale. If head hair is inaccessible, an equivalent quantity of body hair may be used. When mentioning head hair, reference is made to only scalp hair. Body hair pertains to any other hair types (facial, axillary, etc.).

Process Overview

The key stages involved in processing a drug test result in the lab are Accessioning, Screening, Extraction, and Confirmation.

Accessioning relates to the initial handling of a sample entering a laboratory’s system. This encompasses confirming that the sample was properly sealed and dispatched, assigning a random LAN (Laboratory Accessioning Number), and finalizing any extra data entry not covered by an electronic chain of custody system.

Screening represents an initial rapid evaluation for drugs of abuse. Screening offers a cost-effective approach to exclude drug usage in the majority of samples; however, a positive screen needs confirmation to be valid in court. Any samples that exhibit a presumptive positive in Screening demand a subsequent confirmation.

Should a sample test presumptively positive in Screening, further hair is extracted from the initial specimen and made ready for Extraction. During this phase, drugs are isolated from hair at a far lower concentration than in other methods (such as urine or oral fluid), which is why hair drug screening remains a challenging method to conduct.

Any positive Screening result is confirmed through GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing prior to confirmation as required. The full lab process from Accessioning to Confirmation is examined under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 standards accreditation.
