Its main advantage is the extended 90-day detection time, secured through observed collection and nearly impossible to manipulate. Yet for recent drug usage, within 5 days, the urine test remains superior.

What it is, How it works

Hair testing is recognized as an effective method for detecting drug and alcohol consumption. Hair maintains a long-term record of alcohol and other drugs by encapsulating biomarkers within the fibers of the advancing hair strand. When collected near the scalp, the hair can offer up to a roughly three-month detection period for alcohol and other substances. Hair collection is straightforward, moderately resistant to tampering, and simple to transport.

A sample measuring 1.5 inches and consisting of approximately 200 strands of hair (about the size of a #2 pencil) near the scalp yields 100mg of hair, considered the optimal amount for screening and confirmation. For EtG, additional tests, and/or more than 10-panel tests, 150mg of the sample is preferable. We suggest weighing the sample using a jeweler’s scale. If scalp hair is unavailable, a similar quantity of body hair could be collected. The term head hair specifically denotes scalp hair only. Body hair includes all other hair types (facial, underarm, etc.).

Process Overview

The laboratory processing of a drug test result involves four essential steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial incorporation of a sample into a laboratory’s workflow. This process includes ensuring that the sample was appropriately sealed and dispatched, assigning a random LAN (Laboratory Accessioning Number), and fulfilling any supplementary data entry not available through an electronic chain of custody system.

Screening is an initial rapid assessment for substance abuse. Although Screening is a cost-efficient approach to exclude drug use in most samples, a positive result requires confirmation to be valid in legal settings. Any samples with presumptive positive results in Screening necessitate a further confirmation.

Should a sample show presumptive positivity in the Screening phase, additional hair from the original specimen is used for Extraction. Here, drugs are isolated from the hair at a significantly lower concentration compared to other methodologies (e.g., urine or oral fluid), which is why hair drug analysis is the most challenging method to execute.

Confirmation of any positive screen is conducted using GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positive results are washed before confirmation as necessary. The comprehensive laboratory procedure from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair certification and accreditation to ISO / IEC 17025 standards.

  • Resistant to tampering: It is very challenging to manipulate a hair drug test, ensuring greater accuracy in results.

  • Cannot identify recent usage: It takes about 5-7 days before drugs become detectable in hair.
  • Expense: Hair drug tests are typically costlier than other drug testing methods.
  • Result variability: Elements such as hair color and individual differences in hair growth can affect drug metabolite levels in hair.

The manuscripts in this section presented novel approaches, such as multianalyte methods and on-line preparation which can enable rapid analysis and improve thorough forensic investigations.

Clinical Applications of hair testing are witnessed by the insight on environmental tobacco smoke exposure, assessed by hair analysis in a population of students and the study of in utero-exposure to drugs of abuse through neonatal hair analysis. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
