For EtG, additional tests, or panels exceeding ten, a 150mg sample is preferred. It's advisable to use a jeweler's scale for weighing the sample. In cases where scalp hair is unavailable, body hair of an equivalent amount is permissible. "Head hair" specifically denotes scalp hair. "Body hair" includes other types like facial and axillary hair.

Process Overview

There are four primary phases in the laboratory analysis of a drug testing sample: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the early handling of a sample within a lab’s framework. This entails ensuring that the sample was properly sealed and sent, issuing a random LAN (Laboratory Accessioning Number), and finalizing any supplementary data input not provided by an electronic custody chain system.

Screening is a preliminary assessment for substance misuse. While being cost-efficient for ruling out drug consumption in most samples, a positive screen must be substantiated to be legally permissible. Samples that test positive in Screening undergo a further confirmation process.

For samples that show a presumptive positive result in Screening, additional hair is extracted from the same initial sample to prepare for Extraction. Drugs are extracted from hair at a concentration significantly lower than in other methods (e.g., urine or oral fluids), which makes hair drug testing the most challenging method to perform.

Positive screening results are verified using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleansed prior to confirmation as deemed necessary. The entire laboratory procedure, from Accessioning to Confirmation, adheres to the CAP (College of American Pathologists) Hair specifications and the ISO / IEC 17025 accredited standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
