When collected near the scalp, a hair sample can provide a detection period of up to approximately three months for alcohol and other substances. Hair is straightforward to collect, relatively challenging to tamper with, and simple to transport.

A 1.5-inch section consisting of about 200 hair strands (approximately the thickness of a #2 pencil) nearest to the scalp provides 100mg of hair, which is the ideal amount for both screening and confirmation. For EtG, supplementary tests, and/or panels above 10, a 150mg sample is advised. It is suggested to weigh the hair using a jeweler’s scale. If scalp hair is not available, a similar amount of body hair can be gathered. When mentioning head hair, it specifically pertains to scalp hair. Body hair includes all other types (facial, axillary, etc.).

Process Overview

The laboratory steps for processing a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial stage of integrating a sample into a lab’s system. This involves ensuring that the sample was correctly sealed and transported, assigning a random LAN (Laboratory Accessioning Number), and filling in any missing data not provided by an electronic chain of custody process.

Screening consists of a preliminary check for drugs of abuse. Although Screening is a cost-efficient way to exclude drug use on most samples, a positive screen requires confirmation for court suitability. Presumptively positive Screening samples demand secondary confirmation.

If a sample is presumptively positive during Screening, more hair is taken from the original specimen for Extraction. In this phase, substances are extracted from hair at significantly lower concentrations than in other methods (e.g., urine or oral fluid), making hair drug testing the most intricate methodology to execute.

GC/MS, GC/MS/MS or LC/MS/MS is used to confirm any positive results from the Screening stage. Presumptive positive samples are cleansed prior to confirmation as required. The entire process from Accessioning to Confirmation is reviewed following both the CAP (College of American Pathologists) Hair guidelines and the ISO / IEC 17025 accreditation standards.
