For EtG enhancements, and/or assessments exceeding 10 panels, a 150mg sample is advised. It is suggested to measure the sample using a jeweler’s scale. Should scalp hair not be available, the same amount of body hair can be gathered. When discussing head hair, it exclusively pertains to scalp hair. Body hair encompasses all other hair types such as facial, underarm, etc.

Process Overview

The main four actions in laboratory processing of a drug testing result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial intake of a sample into a lab's framework. This process comprises ensuring that the sample is sealed and adequately shipped, allocating a random LAN (Laboratory Accessioning Number), and finalizing any further data entry not supplied by an electronic chain of custody provision.

Screening conducts an initial assessment for substances of abuse. Screening serves as a cost-efficient method to negate drug consumption in most samples; however, a positive screen must be validated to be legally defensible. Any sample presenting presumptive positivity in Screening necessitates further confirmation.

If a sample is found presumptively positive during the Screening phase, additional hair is extracted from the original sample for Extraction. During this step, drugs are isolated from hair at significantly lower concentrations than other testing techniques like urine or saliva, making hair drug screening the most intricate methodology to execute.

Confirmation of any positive screening outcomes is accomplished through GC/MS, GC/MS/MS or LC/MS/MS. All samples with presumptive positivity are cleansed before confirmation, if necessary. The entire lab protocol from Accessioning to Confirmation is inspected under both the CAP (College of American Pathologists) Hair guidelines and the certification to ISO / IEC 17025 criteria.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
