A 1.5-inch segment consisting of around 200 hair strands (comparable to the size of a #2 pencil) nearest to the scalp will provide 100mg of hair, the optimal sample for analysis and confirmation. For EtG, add-ons, and/or tests exceeding 10 panels, 150mg of the sample is advisable. It is suggested to measure the sample using a jeweler’s scale. If scalp hair isn’t accessible, a comparable amount of body hair can be collected. When mentioning head hair, we refer specifically to scalp hair. Body hair denotes all other hair types (facial, axillary, etc.).

Process Overview

The four primary stages in the laboratory processing of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial integration of a sample into a laboratory’s system. This includes checking that the sample was sealed and delivered correctly, assigning an arbitrary LAN (Laboratory Accessioning Number), and completing any further data entry not supplied by an electronic chain of custody system.

Screening comprises an initial swift assessment for drugs of abuse. While Screening is a cost-efficient method to exclude drug consumption for most samples, a positive screen must be verified to be permissible in court. Samples that are presumptively positive in Screening necessitate a subsequent confirmation.

If a sample is presumptively positive during the Screening phase, additional hair is retrieved from the original sample and set for Extraction. During this phase, drugs are extracted from hair at significantly lower concentrations than in other methods (e.g., urine or oral fluid), which accounts for hair drug screening being the most technically challenging to execute.

Confirmation of any positive screening outcome is performed via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleaned before confirmation if needed. The entire laboratory procedure from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and adherence to ISO / IEC 17025 standards.
