What it is, How it works

Hair testing is regarded as an effective method for identifying drug and alcohol use. Hair captures a prolonged record of alcohol and other substance consumption by embedding biomarkers in the fibers of the growing hair shaft. When hairs are gathered close to the scalp, they can reveal up to a roughly 3-month detection span for alcohol and other drugs. Hair is easy to collect, somewhat resistant to adulteration, and convenient to ship.

A 1.5-inch segment of approximately 200 hair strands (around the size of a #2 pencil) nearest to the scalp provides 100mg of hair, which is considered the ideal sample for screening and verification. For EtG, additional tests, and/or panels exceeding 10, a specimen weighing 150mg is advised. We recommend using a jeweler’s scale to measure the specimen. If scalp hair isn't accessible, an equivalent quantity of body hair can be used. When we mention head hair, we mean scalp hair specifically. Body hair pertains to all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result comprises four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning consists of initially processing a sample into a laboratory’s system. This encompasses verifying the proper sealing and shipping of the sample, assigning a random LAN (Laboratory Accessioning Number), and finishing any extra data entry absent from an electronic chain of custody system.

Screening is an initial rapid assessment for drugs of abuse. While Screening is a cost-efficient method to exclude drug usage on most samples, a positive screen must be validated to be permissible in court. Samples that are positively presumed in Screening necessitate an additional confirmation.

For samples presumptively positive during Screening, more hair is extracted from the original specimen and readied for Extraction. During this phase, drugs are separated from hair at a considerably lower concentration compared to other methods (such as urine or oral fluid), making hair drug screening the most challenging method to conduct.

Positive screening results are confirmed using GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples undergo washing before confirmation, as needed. The entire lab process from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 standard accreditation.

Clinical Applications of hair testing are witnessed by the insight on environmental tobacco smoke exposure, assessed by hair analysis in a population of students and the study of in utero-exposure to drugs of abuse through neonatal hair analysis. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
