A 1.5-inch segment consisting of around 200 hair strands (similar in thickness to a #2 pencil) closest to the scalp will yield 100mg, which is the optimal sample size for both screening and confirmation tests. For EtG, additional tests, or tests exceeding 10 panels, a sample weight of 150mg is advised. We suggest using a jeweler’s scale for accurate measurement. If head hair is inaccessible, an equivalent amount of body hair can be used. "Head hair" specifically refers to scalp hair, while "body hair" includes all other hair forms (facial, axillary, etc.).

Process Overview

The procedure for processing a drug test result in the lab includes four principal stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial integration of a sample into the laboratory framework. This process guarantees the sample's seal and shipment integrity, assigns a random LAN (Laboratory Accessioning Number), and completes any outstanding data entry not covered by an electronic chain of custody.

Screening provides an initial rapid check for drugs of abuse. Though Screening is an economical method to exclude drug use for most samples, a positive outcome must be validated for legal admissibility. Samples that appear positive during Screening necessitate further verification.

For samples indicated as positive in Screening, additional hair is extracted from the starting sample to undergo Extraction. In this phase, drugs are separated from the hair at much lower concentrations than in methods like urine or oral fluids, highlighting the challenge in hair drug screening.

Positive screening results undergo Confirmation through GC/MS, GC/MS/MS, or LC/MS/MS methods. Any samples presumed positive are cleansed before confirmation as necessary. The full lab process, from Accessioning to Confirmation, is evaluated following both the CAP (College of American Pathologists) Hair standards and the ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
