What it is, How it works

Hair analysis is increasingly recognized as a robust method for identifying drug and alcohol consumption. Hair captures a long-term record of drug and alcohol use by embedding biomarkers within the hair’s growing fibers. When collected near the scalp, hair can reveal up to a three-month retrospective of substance use. Hair sampling is straightforward, hard to tamper with, and convenient to transport.

A 1.5-inch section consisting of roughly 200 hairs (approximately the diameter of a #2 pencil) close to the scalp yields 100mg, which is an optimal amount for both screening and confirmation. For EtG, add-ons, and tests involving more than 10 panels, 150mg of the sample is advised. We recommend using a jeweler's scale for weighing. If scalp hair is unavailable, an equal quantity of body hair can be obtained. When we mention head hair, we specifically mean scalp hair. Body hair covers all other types (facial, axillary, etc.).

Process Overview

The laboratory analysis of a drug test is comprised of four key phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning marks the first step, where a sample is introduced into the lab's system. This stage involves verifying that the sample was shipped and sealed correctly, assigning a random LAN (Laboratory Accessioning Number), and completing any necessary data entry that hasn't been included by an electronic chain of custody system.

Screening constitutes an initial drug check. While Screening is a budget-friendly method to exclude drug usage in most samples, a positive screening must be authenticated to be acceptable in court. Any samples that test presumptively positive during Screening require subsequent confirmation.

If a sample yields a presumptive positive during Screening, additional hair is extracted from the original sample for the Extraction phase. During this stage, drugs are isolated from the hair at a much lower concentration compared to methods like urine or oral fluid analysis, making hair drug screening the most challenging technique to perform.

Confirmation of any presumptive positive result is accomplished using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing before confirmation as necessary. The complete lab procedure from Accessioning to Confirmation is overseen within the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
