A 1.5-inch segment comprising approximately 200 hair strands (akin to a #2 pencil’s thickness) nearest to the scalp constitutes 100mg of hair, which is the optimal sample for testing and confirmation. For EtG, additional analyses, and/or tests exceeding 10 panels, 150mg of the sample is advised. We suggest measuring the sample on a jeweler’s scale. If scalp hair is absent, an equal amount of body hair may be used. By head hair, we refer solely to scalp hair. Body hair includes all other hair types (facial, axillary, etc.).

Process Overview

The four primary stages in the lab processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial integration of a sample into a lab’s system. This process involves confirming proper sealing and shipment of the sample, assigning a random LAN (Laboratory Accessioning Number), and finishing any extra data entry not included by an electronic chain of custody system.

Screening constitutes a preliminary rapid assessment for drugs of abuse. Although Screening is a budget-friendly method to eliminate drug use in most samples, a positive screen must be confirmed for court admissibility. Samples yielding a presumptive positive in Screening necessitate secondary confirmation.

If a sample yields a presumptive positive during Screening, additional hair is extracted from the initial sample and prepared for Extraction. Here, drugs are removed from hair at a much lower concentration than other methods (e.g., urine or oral fluid), making hair drug testing the most challenging method to execute.

GC/MS, GC/MS/MS, or LC/MS/MS is employed for Confirmation of any positive screening outcomes. All presumptive positive samples undergo washing as required prior to confirmation. The complete lab process from Accessioning to Confirmation is evaluated under the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The joint meeting of the Society of Hair Testing (SoHT) and the Italian Group of Forensic Toxicologists (GTFI) was organized in Verona, June 8–10, 2022, and hosted by the Legal Medicine team of the Verona University Hospital, directed by Franco Tagliaro together with Federica Bortolotti and Rossella Gottardo.

It served as a prominent platform for knowledge exchange and collaboration, facilitating groundbreaking advancements in hair testing and analysis. In this special issue of Drug Testing and Analysis, we aim to showcase the remarkable manuscripts presented at this significant event, shedding light on the latest research and highlighting the future directions in this field.
