For EtG, additional tests, or tests exceeding 10 panels, 150mg of the sample is advised. It is recommended to weigh the sample using a jeweler’s scale. In cases where scalp hair is absent, an equivalent volume of body hair may be used. References to head hair specifically indicate scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The process of drug test result analysis in the lab consists of four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning marks the starting phase of incorporating a sample into a lab's system. This entails ensuring that the sample is properly sealed and dispatched, assigning a random LAN (Laboratory Accessioning Number), and completing any extra data entry absent from an electronic chain of custody system.

Screening performs an initial swift assessment for common drugs of abuse. While it is a cost-efficient method for excluding drug usage in most samples, any sample yielding a positive screen must undergo confirmation to be legally valid. Samples that test presumptively positive during Screening necessitate further confirmation.

Should a sample be presumptively positive during Screening, additional hair is extracted from the original sample and processed for Extraction. During this stage, drugs are removed from hair at significantly lower concentrations than those in alternative methodologies (such as urine or oral fluid), contributing to the complexity of hair drug screening.

Confirmation of any positive screen is achieved through GC/MS, GC/MS/MS, or LC/MS/MS methods. All samples presumed positive are cleansed before confirmation if required. The entire lab procedure from Accessioning to Confirmation is examined under both the CAP (College of American Pathologists) Hair designation and ISO/IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
