What it is, How it works

Sampling hair near the scalp can yield nearly a 3-month assessment period for the presence of alcohol and other drugs. Its collection is straightforward, tampering is relatively challenging, and shipment is effortless.

A 1.5-inch hair sample, containing approximately 200 strands (about the thickness of a #2 pencil) from the scalp area, yields 100mg—ideal for both screening and verification. For tests involving EtG, additional panels, or more than 10 panels, 150mg is advisable. We suggest measuring the sample using a jeweler's scale. If scalp hair isn't available, equivalent body hair can be used. When mentioning head hair, it pertains specifically to scalp hair. Body hair refers to other types (facial, axillary, etc.).

Process Overview

Handling a drug test result in a lab involves four main stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to initially processing a sample within a lab's systems. This incorporates verifying proper sealing and shipment, assigning a random LAN (Laboratory Accessioning Number), and completing data entries that might be missing from an electronic chain of custody system.

Screening consists of a preliminary swift evaluation for drugs. Though it's a cost-effective method to eliminate substance presence in most samples, a positive result requires confirmation for judicial admissibility. Presumptive positive samples in Screening necessitate additional confirmation.

Should a sample show a presumptive positive during Screening, additional hair is taken from the original sample and prepared for Extraction. In this phase, substances are removed from hair at significantly lower concentrations than other methods (such as urine or oral fluid), rendering hair drug screening the most challenging method to execute.

Confirming any positive result in Screening is performed through GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positives undergo cleansing before confirmation if necessary. From Accessioning to Confirmation, the entire lab process is thoroughly evaluated in compliance with both the CAP (College of American Pathologists) Hair distinction and ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
