Recently, the preference for drug testing has shifted towards hair follicle methods. A variety of employers, legal systems, and substance abuse experts now favor hair tests over the traditional urine tests. Hair follicle tests meet the standards for zero-tolerance policies, serving employers, legal authorities, and probation services. Their advantage lies in a 90-day detection capability, being conducted under direct supervision, and resisting tampering more effectively than other tests. Nevertheless, for immediate past use within 5 days, urine testing remains unsurpassed in precision.

What it is, How it works

Hair analysis is regarded as an effective method for identifying drug and alcohol intake. By capturing biomarkers within the fibers of the hair as it grows, hair records a prolonged history of alcohol and other substances. When collected near the scalp, it can offer an approximately 3-month detection timeframe for various substances. Hair collection is straightforward, harder to tamper with, and easily transportable.

Gathering a 1.5-inch sample consisting of about 200 hair strands (about the thickness of a #2 pencil) close to the scalp provides 100mg of hair, which is the optimal quantity for examination and verification. For EtG testing, additional panels beyond 10, or for extended tests, 150mg of hair is recommended. We suggest measuring the sample with a jeweler’s scale. If there isn't enough scalp hair, an equivalent amount of body hair can be collected. "Head hair" indicates scalp hair only, whereas "body hair" includes all other types (such as facial and axillary hair).

Process Overview

The laboratory procedure for a drug test result consists of Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the process of integrating a sample into a lab’s workflow, ensuring it was properly sealed and transported, assigning a random LAN (Laboratory Accessioning Number), and completing any extra data input absent from an electronic custody record.

Screening is a preliminary examination to detect drugs. While it is cost-effective in ruling out drug use for most samples, any positive screen must be verified for legal admissibility. Presumptive positive samples in Screening require further Confirmation.

If a sample shows a preliminary positive in Screening, more hair is extracted from the original sample and prepared for Extraction. Drugs are extracted from hair at a lower concentration than in other methods (like urine or oral fluid), making hair drug screening the most challenging technique.

Confirmation for any initial positive screening is performed through GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples undergo washing before Confirmation as required. The whole laboratory procedure from Accessioning to Confirmation is vetted under both the CAP (College of American Pathologists) Hair classification and the ISO / IEC 17025 accreditation.

Advantages of hair drug testing:

Note: Although often called "hair follicle tests", the analysis occurs on the hair strand and not on the follicle beneath the scalp.

The joint meeting of the Society of Hair Testing (SoHT) and the Italian Group of Forensic Toxicologists (GTFI) was organized in Verona, June 8–10, 2022, and hosted by the Legal Medicine team of the Verona University Hospital, directed by Franco Tagliaro together with Federica Bortolotti and Rossella Gottardo.

It served as a prominent platform for knowledge exchange and collaboration, facilitating groundbreaking advancements in hair testing and analysis. In this special issue of Drug Testing and Analysis, we aim to showcase the remarkable manuscripts presented at this significant event, shedding light on the latest research and highlighting the future directions in this field.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
