A segment measuring 1.5 inches with around 200 hair strands (comparable to a #2 pencil) from the scalp offers 100mg of hair, which is optimal for analysis and verification. For EtG, add-ons, or tests exceeding 10 panels, 150mg of the sample is advised. Using a jeweler’s scale to weigh the specimen is recommended. If scalp hair is unavailable, a similar quantity of body hair can be used. "Head hair" pertains to scalp hair only, while "body hair" comprises all other hair types (such as facial, axillary, etc.).

Process Overview

The primary steps in the lab processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the preliminary processing of a sample within the lab’s framework. This process ensures the sample was sealed and delivered correctly, assigns a random LAN (Laboratory Accessioning Number), and completes any additional data entry not covered by an electronic chain of custody system.

Screening offers an initial rapid assessment for drugs. Although Screening is an economical approach to dismiss drug use in most samples, a positive results necessitate confirmation for court admissibility. Any samples presumed positive in Screening must undergo secondary confirmation.

If a sample is presumed positive in Screening, additional hair from the sample is prepared for Extraction, where drugs are extracted at much lower concentrations compared to other methods (e.g., urine or saliva), highlighting the complexity of hair drug screening.

Confirmation of positive screen results is performed using GC/MS, GC/MS/MS, or LC/MS/MS. Presumed positive samples are cleaned before confirmation if necessary. The entire lab process from Accessioning to Confirmation undergoes review under the CAP Hair classification and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
