If scalp hair is not available, an equivalent quantity of body hair can be used. When mentioning head hair, we refer exclusively to scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The primary stages in processing a drug test result in a lab are Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial integration of a sample into the laboratory’s system. This stage ensures the sample is sealed and shipped correctly, assigns a random LAN (Laboratory Accessioning Number), and fills in any supplementary data not included in an electronic chain of custody system.

Screening serves as a preliminary assessment for drugs of abuse. Although Screening is a cost-efficient approach to exclude drug usage in most samples, a positive screen must be verified for court admissibility. Any presumptive positive samples from Screening necessitate a secondary confirmation.

If a sample tests presumptively positive during Screening, additional hair is extracted from the original sample and readied for Extraction. During this phase, drugs are isolated from hair at significantly lower concentrations than other methods (e.g., urine or oral fluid), thus making hair drug screening the most challenging method to execute.

Positive screening results are confirmed through GC/MS, GC/MS/MS, or LC/MS/MS. Any samples testing presumptively positive are cleansed before confirmation as required. The entire lab process from Accessioning to Confirmation undergoes review under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
