A 1.5-inch section consisting of approximately 200 hair strands (similar in size to a #2 pencil) close to the scalp provides 100mg of hair, which is the optimal sample size for screening and confirmation. For EtG, additional tests, and/or panels exceeding 10, a specimen of 150mg is advised. We suggest using a jeweler’s scale to weigh the specimen. If scalp hair is not available, an equivalent amount of body hair can be collected. When we mention head hair, it solely refers to scalp hair. Body hair includes all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial handling of a sample in a laboratory's system. This step includes verifying that the sample was appropriately sealed and dispatched, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry not supplied by an electronic chain of custody system.

Screening entails a quick initial assessment for drugs of abuse. While screening provides a cost-effective method to exclude drug usage in most samples, a positive screen must undergo confirmation to be acceptable in court. Any samples that are presumptively positive in this stage require a secondary confirmation.

If a sample tests presumptively positive during Screening, more hair is taken from the initial specimen and prepared for Extraction. During this stage, drugs are extracted from hair at significantly lower concentrations compared to other methods like urine or oral fluid, making hair drug screening one of the most challenging methodologies to execute.

Verification of any positive screening result is executed through GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing before confirmation as necessary. The entire laboratory progression, from Accessioning to Confirmation, is assessed under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.
