What it is, How it works

Hair testing has emerged as an effective method for identifying the use of drugs and alcohol. Hair captures a comprehensive history of alcohol and other substances by sealing biomarkers within the strands as hair grows. When collected close to the scalp, hair can provide an approximate three-month detection window for alcohol and other substances. Hair is straightforward to gather, somewhat challenging to tamper with, and uncomplicated to transport.

A 1.5-inch segment of around 200 hair strands (comparable to the thickness of a #2 pencil) nearest to the scalp will yield 100mg, an optimal sample for both screening and confirmation. For EtG, add-ons, and/or tests exceeding 10 panels, 150mg of hair is advised. It is suggested to weigh the sample using a jeweler’s scale. If head hair is unavailable, an equivalent amount of body hair may be used. By head hair, we refer solely to scalp hair, whereas body hair encompasses all other hair types (such as facial, axillary, etc.).

Process Overview

The laboratory analysis of a drug test result comprises four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial processing of a specimen into a laboratory's system. This process verifies that the specimen was correctly sealed and shipped, assigns an arbitrary LAN (Laboratory Accessioning Number), and completes any additional data entry omitted by an electronic chain of custody system.

Screening consists of a preliminary rapid assessment for drugs of abuse. While Screening is an economical approach to eliminate drug usage in most samples, a positive screening outcome requires confirmation to be defensible in legal settings. Any samples with a presumptive positive result in Screening necessitate a follow-up confirmation.

If a specimen yields a presumptive positive in the Screening phase, further hair is extracted from the initial sample and readied for Extraction. In this phase, drugs are isolated from hair at significantly lower concentrations than in alternative methodologies (such as urine or oral fluid), which is why hair drug screening is notably complex to conduct.

Confirmation of any positive screening outcome is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positives undergo washing prior to confirmation as needed. The entire laboratory procedure from Accessioning to Confirmation is subject to review under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The impact of hair treatments on the oxidation/decomposition of drugs in hair was also explored, as the long-term fate of drugs incorporated in the keratin matrix. Clinical Applications of hair testing are witnessed by the insight on environmental tobacco smoke exposure, assessed by hair analysis in a population of students and the study of in utero-exposure to drugs of abuse through neonatal hair analysis. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
