A sample of 1.5 inches with roughly 200 strands, akin in size to a #2 pencil, provides 100mg of hair, which is the optimal sample size for analysis and verification. For EtG and tests exceeding 10 panels, a 150mg sample is advised. We suggest using a jeweler’s scale for precise weighing. If scalp hair isn't available, the equivalent amount of body hair can be used. When mentioning head hair, it refers specifically to scalp hair. Body hair includes all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing for drug test results includes four major steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step of entering a sample into the lab’s database, ensuring it was sealed and shipped correctly, assigning a random LAN (Laboratory Accessioning Number), and completing other data requirements not handled by an electronic chain of custody.

Screening provides a preliminary check for drug abuse presence. Though effective in excluding drug use for most samples, a positive result requires confirmation to be legally valid. Any sample testing presumptively positive must undergo a further confirmation.

For samples testing positive in Screening, additional hair is taken from the original sample for Extraction. During Extraction, drugs are separated from hair at a much lower concentration than other methods (e.g., urine or oral fluid), making hair drug screening the toughest to carry out.

Positive screening is confirmed through GC/MS, GC/MS/MS, or LC/MS/MS techniques. All positive samples are cleaned prior to confirmation as necessary. The complete lab procedure from Accessioning to Confirmation adheres to the CAP (College of American Pathologists) Hair criteria and meets the ISO/IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
