Process Overview

The laboratory processing of a drug test outcome involves four key phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning embodies the primary processing of a specimen into the lab's system. This phase consists of confirming proper sealing and shipment of the sample, allocating a random LAN (Laboratory Accessioning Number), and filling in any absent data not available through an electronic chain of custody system.

Screening entails an initial rapid assessment for substances of abuse. While Screening effectively rules out drug consumption in most samples, a positive screen demands verification for legal admissibility. Samples that are presumptively positive during Screening necessitate further confirmation.

If Screening shows a presumptive positive, additional hair is sourced from the original sample for Extraction. Confirmation of any positive screen results is executed using GC/MS, GC/MS/MS, or LC/MS/MS. Before confirmation, every presumptively positive sample undergoes cleansing as required. The entire cycle from Accessioning to Confirmation undergoes review under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 quality standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field.
