In the absence of scalp hair, a comparable quantity of body hair may be utilized. “Head hair” refers exclusively to scalp hair, while “body hair” encompasses all other hair kinds (e.g., facial, axillary, etc.).

Process Overview

The primary four stages in the testing lab for drug result processing comprise Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves initially integrating the sample into the lab's system. This step ensures the sample was sealed and transported correctly, assigns a random LAN (Laboratory Accessioning Number), and completes any extra data not provided by an electronic chain of custody system.

Screening consists of a preliminary rapid assessment for substances of abuse. Although Screening offers a cost-effective method to eliminate drug use in most samples, a positive screen requires confirmation for courtroom acceptability. Samples that yield a presumptive positive in Screening mandatorily need further confirmation.

If presumptive positivity occurs in the Screening, additional hair is taken from the original sample and readied for Extraction. Here, substances are extracted at much lower concentrations in comparison to other methodologies (for example, urine or oral fluid), contributing to the complexity of hair drug testing methodologies.

Confirmation for any positive screening outcome is performed using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All presumptive positive samples undergo necessary washing before confirmation. The entire process from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and compliance with ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
