Gathering a 1.5-inch section consisting of about 200 hairs (approximately the circumference of a #2 pencil) nearest the scalp yields 100mg, which is the optimal amount for screening and confirmation. For EtG tests, additional panels, or tests over 10 panels, 150mg of the sample is advised. Using a jeweler’s scale to weigh the specimen is suggested. If scalp hair isn't available, an equivalent quantity of body hair can be used. The term "head hair" refers solely to scalp hair, while "body hair" encompasses all other varieties (facial, axillary, etc.).

Process Overview

The laboratory procedure for a drug test result includes Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial stage where a sample is entered into the laboratory’s system. This step confirms the sample’s seal and proper shipment, allocates a random LAN (Laboratory Accessioning Number), and fulfills any data entry overlooked by an electronic chain of custody system.

Screening comprises a preliminary assessment for drugs of abuse. While offering a cost-efficient method to exclude drug usage for most specimens, a positive screen must undergo confirmation to be legally valid. Samples testing presumptively positive during Screening necessitate a secondary confirmation.

For a presumptive positive result at the Screening phase, additional hair is extracted from the original sample and readied for Extraction. In this process, drugs are separated from the hair at a significantly reduced concentration compared to other methods (e.g., urine or oral fluid), making hair drug screening one of the more challenging methodologies.

Confirmation of a positive Screening result is achieved through GC/MS, GC/MS/MS, or LC/MS/MS. Before confirmation, presumptive positive samples undergo washing as necessary. The entire laboratory process from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation as well as accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
