Hair offers a prolonged record of alcohol and other substances by capturing biomarkers within the strands of the hair as it grows. When gathered near the scalp, hair can indicate substance use within roughly a three-month period. It's straightforward to collect, relatively challenging to tamper with, and simple to send.

A segment measuring 1.5 inches from about 200 hairs (around the diameter of a #2 pencil) near the scalp yields 100mg of hair, the ideal quantity for both initial tests and confirmation processes. For EtG, additional tests, and/or screenings beyond 10 panels, a sample of 150mg is recommended. We suggest weighing the hair sample on a jeweler’s scale. If scalp hair is not available, a comparable quantity of body hair may be used. When discussing head hair, we specifically mean scalp hair. Body hair encompasses all other kinds (such as facial and axillary hair).

Process Overview

The four principal stages in processing a drug test in the lab are Accessioning, Screening, Extraction, and Confirmation.

Accessioning refers to the first stage of placing the sample into the lab system. This process includes checking that the sample was properly sealed and transported, assigning a random LAN (Laboratory Accessioning Number), and filling in any extra data not captured by an electronic chain of custody system.

Screening is a preliminary assessment for substances of abuse. Although Screening is an economical approach to discard drug usage for most samples, a positive result in Screening necessitates confirmation for legal validity. Samples that show a presumptive positive result in Screening must undergo a secondary confirmation.

If a sample shows a presumptive positive in the Screening phase, additional hair is extracted from the original sample for the Extraction process. In this phase, substances are isolated from the hair at much lower concentrations than in other methods (e.g., urine or oral fluids), making hair analysis the most complex method to carry out.

Confirmation of any positive result is performed through GC/MS, GC/MS/MS, or LC/MS/MS. Any presumptive positive samples are rinsed before confirmation when needed. The complete laboratory procedure from Accessioning to Confirmation is reviewed under the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
