A 1.5-inch piece comprising around 200 hairs (about the width of a #2 pencil) closest to the scalp yields 100mg of hair, which is the optimal sample for screening and confirmation. For EtG, additional testing, and/or tests beyond 10 panels, 150mg of the specimen is suggested. It is advisable to weigh the specimen using a jeweler’s scale. If scalp hair is not available, an equivalent amount of body hair can be collected. The term head hair refers exclusively to scalp hair. Body hair includes all other hair types (facial, axillary, etc.).

Process Overview

The four key stages in lab processing of a drug test outcome are Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial integration of a sample into a lab’s system. This step verifies the sample is sealed and shipped correctly, assigns a unique LAN (Laboratory Accessioning Number), and completes any extra data entry not covered by an electronic chain of custody system.

Screening involves an initial swift check for substances of abuse. Though it is a cost-efficient way to rule out substance usage for most samples, a positive result requires confirmation for courtroom validity. Samples with an initial positive result in Screening need a follow-up confirmation.

When a sample has a positive preliminary result in the Screening phase, additional hair is taken from the initial sample and prepared for Extraction. In this phase, substances are isolated from hair at a much lower concentration than in other methods (e.g., urine or oral fluid), making hair drug analysis the most complex method to execute.

Affirmation of any positive screen result is conducted through GC/MS, GC/MS/MS, or LC/MS/MS. All samples presumed positive are cleaned before confirmation if required. The full lab procedure from Accessioning to Confirmation undergoes review under both the CAP (College of American Pathologists) Hair criteria and the accreditation to ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
