For EtG, additional tests, and/or assessments exceeding 10 panels, a 150mg sample is advised. Utilizing a jeweler’s scale for weighing the specimen is suggested. If scalp hair cannot be obtained, a comparable volume of body hair may be used. “Head hair” references to scalp hair exclusively, whereas “body hair” encompasses all other types like facial, axillary, etc.

Process Overview

The laboratory drug testing procedure consists of four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the preliminary integration of a sample into the lab’s system. This phase ensures the sample was properly sealed and dispatched, assigns it a random LAN (Laboratory Accessioning Number), and completes any outstanding data entry not captured by an electronic custody platform.

Screening is a preliminary assessment for drug presence. Though Screening offers a cost-effective method to exclude drug use in most cases, a positive result necessitates confirmation to be legally viable. Samples that show presumptive positivity in Screening must undergo additional confirmation.

For samples showing presumptive positivity during Screening, additional hair is sourced from the original specimen for Extraction. In this stage, substances are isolated from the hair at concentrations much lower than those found using alternative methods (e.g., urine or saliva), contributing to the complexity of hair drug screening procedures.

Confirmation for any positive Screening outcome is performed using GC/MS, GC/MS/MS, or LC/MS/MS. Presumptively positive samples are washed before confirmation as required. The entire processing from Accessioning to Confirmation is scrutinized under the CAP (College of American Pathologists) Hair classification and the ISO/IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
