What it is, How it works

Hair testing is increasingly recognized as an effective means of detecting drug and alcohol consumption. Hair captures a long-term record of alcohol and other substances by embedding biomarkers within its fibers as it grows. When hair is collected near the scalp, it can offer a detection period of up to about 3 months for alcohol and other substances. This type of hair is easy to obtain, less prone to tampering, and convenient to send.

A 1.5-inch section consisting of approximately 200 hair strands (roughly the width of a #2 pencil) taken closest to the scalp will yield 100mg of hair, which is the optimal amount for both screening and verification. When conducting EtG, supplementary tests, and/or more than 10 panels, 150mg of the sample is advisable. We suggest measuring the sample with a jeweler’s scale. If scalp hair isn't available, a similar quantity of body hair can be collected. When mentioning head hair, we only mean scalp hair. Body hair includes all other types (facial, axillary, etc.).

Process Overview

The laboratory procedure for processing a drug test involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step of incorporating a sample into a lab’s system. It comprises confirming that the sample was sealed and dispatched correctly, allocating a random LAN (Laboratory Accessioning Number), and finishing any extra data entry not included in an automated chain of custody system.

Screening consists of an expedited check for substances typically abused. Although Screening is a cost-efficient approach to exclude drug use for most samples, a positive screen necessitates confirmation to be valid in legal settings. Samples with presumptively positive results in Screening require additional verification.

When a sample yields a potentially positive result in the Screening phase, more hair is extracted from the primary sample and readied for Extraction. This step involves isolating drugs from hair at considerably lower concentrations compared to other techniques (such as urine or oral fluid), which is why hair drug screening is the most complex technique to carry out.

Verification of any positive screening outcome is performed using GC/MS, GC/MS/MS, or LC/MS/MS methodologies. All samples presumed positive undergo washing before confirmation if needed. The entire lab process from Accessioning to Confirmation is evaluated according to both the CAP (College of American Pathologists) Hair classification and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
