What it is, How it works

Hair analysis is increasingly recognized as an effective method for identifying drug and alcohol consumption. Hair provides an extended record of alcohol and other substances by encasing biomarkers in the strands of hair as it grows. When gathered near the scalp, hair can offer a detection period of up to roughly 3 months for alcohol and other drugs. Hair is easy to collect, somewhat challenging to tamper with, and convenient to transport.

A 1.5-inch portion of about 200 hair strands (comparable to a #2 pencil) nearest to the scalp will yield 100mg of hair, the perfect amount for analysis and verification. For EtG, add-ons, and/or tests beyond 10 panels, 150mg of the sample is advised. We suggest weighing the specimen using a jeweler’s scale. If scalp hair is not available, a similar amount of body hair may be used. References to head hair specifically mean scalp hair. Body hair includes all other hair types (facial, axillary, etc.).

Process Overview

The essential stages in the laboratory analysis of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the preliminary processing of a sample into a lab’s system. This process checks that the sample was sealed and transported correctly, assigns a random LAN (Laboratory Accessioning Number), and completes any missing data entry not available through an electronic chain of custody system.

Screening is a fast initial review for drugs of abuse. While Screening is a budget-friendly method to exclude drug use on the majority of samples, any positive screen must be validated for it to be credible in legal settings. Samples that are preliminarily positive in Screening must undergo a secondary confirmation.

If a sample is preliminarily positive in the Screening phase, additional hair is taken from the initial sample for Extraction. At this phase, drugs are extracted from hair at significantly lower concentrations than other methods (e.g., urine or oral fluid), hence hair drug screening is the most challenging method to execute.

Positive screening results undergo Confirmation using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptively positive samples are cleansed before confirmation as required. The full lab process from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
