For EtG additions and/or tests exceeding 10 panels, 150mg is advised. It's best to weigh the hair on a jeweler's scale. If scalp hair isn't available, body hair in equivalent quantity may be used. 'Head hair' refers strictly to scalp hair, whereas 'body hair' includes all other hair types (facial, armpit, etc.).

Process Overview

The drug test laboratory procedure consists of four fundamental steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the primary handling of a sample into the lab’s framework. This step involves confirming correct sealing and shipping of the sample, assigning a unique LAN (Laboratory Accessioning Number), and finalizing any additional data input not covered by an electronic custody system.

Screening serves as an initial rapid assessment for substance misuse. Although Screening efficiently rules out drug presence in the majority of cases, any positive findings necessitate confirmation for legal admissibility. Presumptive positives identified in Screening must undergo further verification.

A sample flagged as positive during Screening necessitates additional hair extraction from the initial sample for further processing in the Extraction stage. Here, substances are isolated from hair at significantly lower concentrations compared to other testing methods (like urine or saliva). This complexity makes hair testing particularly challenging.

Verification of positive screening outcomes is carried out using GC/MS, GC/MS/MS, or LC/MS/MS techniques. Presumptive positive samples receive a wash before confirmation when required. The lab process spanning Accessioning to Confirmation undergoes scrutiny under both the CAP (College of American Pathologists) designation and ISO/IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
