If scalp hair is absent, a comparable quantity of body hair can be used. References to head hair pertain solely to scalp hair, while body hair refers to all other types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result primarily consists of four stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial integration of a sample into a laboratory’s system, including verification of proper sealing and shipment, allocation of a random LAN (Laboratory Accessioning Number), and completion of any additional data entry not provided by an electronic chain of custody system.

Screening is a preliminary rapid assessment for drug misuse. Although cost-effective for ruling out drug use in most samples, a positive screening must be further validated for court acceptance. Samples that test presumptively positive during Screening require subsequent confirmation.

Should a sample be presumptively positive during Screening, additional hair is taken from the initial specimen and prepped for Extraction. During this phase, drugs are isolated from the hair at considerably lower concentrations than with other methods (e.g., urine or oral fluid), rendering hair drug analysis as the most challenging methodology.

Any positive screening results undergo Confirmation through GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples are cleansed prior to confirmation as necessary. The entire lab process from Accessioning to Confirmation is evaluated under the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.
