Process Overview

The laboratory evaluation of a drug test consists of four primary phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the sample's initial integration into the lab’s system. This step includes ensuring the sample is correctly sealed and shipped, generating a random LAN (Laboratory Accessioning Number), and completing any further data input that an electronic chain of custody does not provide.

Screening is a preliminary quick assessment for substances of abuse. Although Screening is a budget-friendly approach to exclude drug use in most samples, any positive results require confirmation to be valid in legal proceedings. Samples showing presumptive positives during Screening must undergo a secondary verification.

Should a sample present a presumptive positive during Screening, additional hair is drawn from the initial specimen for Extraction. During this process, drugs are extracted from hair at a much lesser concentration than other methods (e.g., urine or oral fluid), making hair drug screening the most challenging method to perform.

Confirmation of any positive Screening outcomes is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples deemed presumptive positive are cleaned before confirmation if necessary. The entire lab process from Accessioning to Confirmation is evaluated under the CAP (College of American Pathologists) Hair specification and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
