When taken from near the scalp, hair can reveal drug or alcohol consumption over a span of approximately 3 months. It is easy to collect, challenging to tamper with, and simple to transport.

A sample measuring 1.5 inches of around 200 hair strands (roughly the thickness of a #2 pencil) from close to the scalp offers 100mg, the preferred quantity for analysis and verification. For EtG, additional tests, or tests that include more than 10 panels, 150mg is suggested. Evaluating the sample on a jeweler’s scale is advised. If there is no scalp hair available, an equivalent amount of body hair can be gathered. The term "head hair" is exclusively for scalp hair, while "body hair" includes all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug analysis consists of four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the sample's entry into a lab's system. This phase ensures the sample was properly sealed and sent, assigns a random LAN (Laboratory Accessioning Number), and completes any data entry not supplied by an electronic chain of custody.

Screening is a preliminary check for drug usage. Although cost-effective in ruling out drug use in most cases, a confirmed positive screening is necessary for legal acceptance. Samples with presumptive positive results during Screening require secondary confirmation.

With a presumptive positive from Screening, more hair is extracted from the initial sample and prepared for Extraction. Here, drugs are isolated from hair at significantly lower concentrations than other methods (e.g., urine or oral fluid), which is why hair drug analysis is the most complex methodology to conduct.

Confirmation of any presumptive positive result is done using GC/MS, GC/MS/MS, or LC/MS/MS methods. All presumptively positive samples undergo washing before confirmation if necessary. The full lab procedure from Accessioning to Confirmation is scrutinized under CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 standards accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
