The process of collecting hair is straightforward, it is relatively hard to tamper with, and convenient for transportation.

Collecting a 1.5-inch segment from roughly 200 hairs (similar in size to a #2 pencil) nearest the scalp yields approximately 100mg, an optimal amount for both screening and confirming tests. For EtG, additional requests, and tests exceeding 10 panels, 150mg of hair is advisable. Use a jeweler’s scale to verify the specimen's weight. If scalp hair is not available, an equivalent quantity of body hair can be used. Note that head hair specifically refers to scalp hair, whereas body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The laboratory drug test result processing involves four principal stages: Accessioning, Screening, Extraction, and Confirmation.

In the Accessioning phase, the sample undergoes initial integration into the lab’s system. This step includes ensuring the sample was correctly sealed and dispatched, assigning a unique Laboratory Accessioning Number (LAN), and completing any missing data entries outside those supported by an electronic custody chain.

Screening conducts a preliminary assessment for drug presence. Although a cost-effective method to negate drug use in most instances, a positive screening must be validated for court acceptance. Samples that test positive during Screening must undergo a follow-up confirmation.

Should a sample test positive during Screening, additional hair is retrieved for Extraction. This phase involves extracting drugs from hair at concentrations significantly lower than other testing methods (e.g., urine or oral fluids), highlighting the complexity of hair drug testing methodologies.

Confirmation of any initial positive results utilizes GC/MS, GC/MS/MS, or LC/MS/MS techniques. Presumptive positive samples undergo washing before confirmation if necessary. The entire lab process, from Accessioning to Confirmation, aligns with the CAP (College of American Pathologists) Hair standards and ISO / IEC 17025 accreditation.

The joint meeting of the Society of Hair Testing (SoHT) and the Italian Group of Forensic Toxicologists (GTFI) was organized in Verona, June 8–10, 2022, and hosted by the Legal Medicine team of the Verona University Hospital, directed by Franco Tagliaro together with Federica Bortolotti and Rossella Gottardo.

It served as a prominent platform for knowledge exchange and collaboration, facilitating groundbreaking advancements in hair testing and analysis. In this special issue of Drug Testing and Analysis, we aim to showcase the remarkable manuscripts presented at this significant event, shedding light on the latest research and highlighting the future directions in this field.

Novel Analytical Techniques: The Verona meeting witnessed the unveiling of cutting-edge analytical techniques, offering enhanced sensitivity, selectivity, and accuracy for drug testing. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
