Typically, a sample requires 1.5 to 2 inches of hair from the head or other body parts. Hair tests can determine drug, alcohol/ETG use, or be tailored for specific tests.

Over the years, the inclusion of hair follicle drug tests in drug screening processes has surged. Many employers, judicial systems, and Substance Abuse professionals opt for hair drug tests over conventional urine tests. This test is crucial for employers with stringent drug policies, courts, and parolees. Its main advantage is a prolonged detection window for drug use, usually up to 90 days, collected under supervision, making it highly cheat-resistant. Nonetheless, urine tests remain unparalleled for detecting recent drug use within the past five days.

What it is, How it works

Hair tests are recognized as an effective method for uncovering drug and alcohol consumption. Hair captures a prolonged record of alcohol and other drug intake by embedding biomarkers within its fibers. When harvested from near the scalp, hair can indicate substance use over approximately a three-month span. Collection of hair is straightforward, tricky to tamper with, and simple to transport.

A 1.5-inch hair sample comprising roughly 200 strands (akin to the thickness of a #2 pencil) closest to the scalp yields 100mg, which is the optimal amount for both screening and confirmation. For tests including EtG, additional parameters, and those exceeding 10 panels, 150mg of hair is advised. We suggest measuring the sample with a jeweler’s scale. Should scalp hair be absent, a similar quantity of body hair may be used. When referencing head hair, this pertains solely to scalp hair. Body hair encapsulates all other hair types (such as facial, axillary, etc.).

Process Overview

The lab processing of drug test outcomes involves four primary phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the primary integration of a sample into a lab’s database. This process ensures the sample was appropriately sealed and transported, assigns a random LAN (Laboratory Accessioning Number), and completes any extra data entry absent in an electronic chain of custody system.

Screening provides an initial rapid assessment for substance abuse. Although Screening is a cost-efficient method to dismiss drug use in the majority of cases, a positive result requires confirmation to hold up in legal settings. Samples that present as presumptively positive during Screening necessitate further confirmation.

In instances of presumptive positivity in Screening, additional hair is processed from the original sample and prepped for Extraction. During Extraction, substances are retrieved from hair at a significantly lower concentration compared to alternative methods (such as urine or oral fluid), highlighting hair drug screening as the most challenging method to execute.

Confirmation of any positive screening result is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples that initially appear positive undergo washing before confirmation as necessary. The entire lab process from Accessioning to Confirmation is evaluated in compliance with both the CAP (College of American Pathologists) Hair guidelines and the accreditation to ISO / IEC 17025 standards.

The benefits of hair drug testing include:

  • Extended detection window: Hair tests can reveal drug use up to 90 days, unlike urine tests which offer a shorter detection period.
  • Resistance to tampering: It is extremely hard to deceive a hair drug test, resulting in more reliable findings.
  • Provision of historical patterns: It can illustrate a timeline of drug use, rather than just recent activity.

Drawbacks:

  • Inability to detect recent ingestion: It takes roughly 5-7 days for drugs to become traceable in hair.
  • Expense: Hair drug tests tend to be pricier compared to other testing methods.
  • Result variability: Elements such as hair color and individual hair growth differences can influence drug metabolite concentration in hair.

Note: While often termed "hair follicle tests", the analysis is performed on the hair strand itself and not the follicle beneath the scalp.
