What it is, How it works

Hair analysis is widely regarded as an effective method for identifying drug and alcohol consumption. Hair offers an extensive history of substances by trapping biomarkers within the fibers of the hair as it grows. When gathered near the scalp, it can present up to a three-month history of substance use. Hair collection is straightforward, relatively tamper-resistant, and simple to transport.

A sample measuring 1.5 inches long with around 200 hair strands (roughly the girth of a #2 pencil) taken nearest to the scalp provides 100mg, ideal for screening and confirmation. For EtG, add-ons, and/or tests beyond 10 panels, a 150mg sample is advised. We suggest using a jeweler's scale to weigh the hair. If scalp hair is unavailable, a comparable amount of body hair may be utilized. When mentioning head hair, it specifically refers to scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The four key phases in laboratory processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial intake of a sample into the lab’s system. It involves verifying that the sample was sealed and shipped correctly, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry not covered by an electronic chain of custody system.

Screening offers an initial quick evaluation for drugs of abuse. While it is a cost-efficient way to eliminate drug usage on most samples, a positive screen requires confirmation to be valid in legal proceedings. Any samples presumed positive in Screening must undergo a secondary confirmation.

If a sample shows a presumptively positive result in Screening, additional hair is extracted from the initial sample for the Extraction phase. During this stage, drugs are drawn from hair at a much lower concentration compared to other methods (e.g., urine or oral fluid), making hair drug screening a highly challenging methodology.

Confirmation of any screening positive result is performed through GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples are washed before confirmation as necessary. The entire lab process from Accessioning to Confirmation is conducted under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
