What it is, How it works

Hair examinations are now acknowledged as an effective method for identifying drug and alcohol consumption. Hair captures a prolonged history of alcohol and drug usage by encapsulating biomarkers within the fibers of the emerging hair strand. When gathered near the scalp, hair can offer nearly a 3-month detection period for alcohol and other substances. Hair is easy to collect, somewhat challenging to tamper with, and simple to transport.

A 1.5-inch segment consisting of approximately 200 hair strands (roughly the diameter of a #2 pencil) from closest to the scalp will yield 100mg of hair, which is the ideal sample for screening and confirming results. For EtG, supplementary tests, and/or panels exceeding 10, a specimen of 150mg is suggested. We advise weighing the sample with a jeweler’s scale. If scalp hair isn’t accessible, a comparable quantity of body hair may be used. When mentioning head hair, we specifically mean scalp hair. Body hair refers to all other types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result comprises four main phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning denotes the initial phase of integrating a sample into the laboratory’s system. This involves validating that the specimen was properly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and completing any supplementary data entry absent from an electronic chain of custody system.

Screening constitutes an initial swift assessment for substances of abuse. Although Screening is cost-effective for ruling out drug use in the majority of samples, a positive result necessitates confirmation to be court-admissible. Presumptively positive Screening samples require additional confirmation.

Should a sample test positive presumptively during the Screening stage, more hair is extracted from the original specimen for Preparation in Extraction. During this phase, drugs are extracted from hair at significantly lower concentrations than other methods (e.g., urine or oral fluid), explaining why hair drug screening is the most challenging method to conduct.

Confirmation of any positive screening outcome is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing before confirmation as necessary. The comprehensive laboratory procedure from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

  • Difficult to manipulate: It's very challenging to deceive a hair drug test, increasing the accuracy of results.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
