Hair is easy to gather, quite challenging to tamper with, and straightforward to transport.

A sample of 1.5 inches consisting of approximately 200 hair strands (comparable to a #2 pencil in size) near the scalp provides around 100mg of hair, an ideal amount for both screening and confirmation. For expanded tests such as EtG, add-ons, and tests over 10 panels, a 150mg sample is advised. It's recommended to weigh the sample using a jeweler’s scale. If scalp hair isn't available, an equal quantity of body hair may be gathered. When the term head hair is used, it refers specifically to scalar hair, whereas body hair encompasses all other hair types (e.g., facial, axillary).

Process Overview

The primary steps in the lab processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial sample processing into the lab's system. This procedure verifies the proper sealing and shipping of the sample, assigns a random LAN (Laboratory Accessioning Number), and completes any additional data entry not covered by an electronic chain of custody system.

Screening performs a preliminary assessment for substances of abuse. Although cost-effective for ruling out drug usage in most samples, a positive screen must be confirmed for legal admissibility. Presumptively positive samples in Screening do necessitate a secondary confirmation.

Should a sample be presumptively positive during the Screening phase, additional hair is extracted from the initial specimen and prepared for Extraction. In this phase, substances are removed from hair at notably lower concentrations than in other techniques (e.g., urine or oral fluid), which explains why hair drug screening is the most challenging methodology.

Confirmation of any positive Screening outcome is carried out through GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing prior to confirmation if necessary. The entire lab process from Accessioning to Confirmation is reviewed under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
