A 1.5-inch segment of approximately 200 hair fibers (roughly equivalent to a #2 pencil size) closest to the scalp provides 100mg of hair, which is the optimal sample size for both screening and confirmation. For EtG, additional tests, and/or panels above ten, 150mg of specimen is advisable. We suggest weighing the sample using a jeweler’s scale. If scalp hair cannot be obtained, an equivalent amount of body hair is acceptable. When mentioning head hair, we mean scalp hair exclusively. Body hair encompasses other hair types (facial, axillary, etc.).

Process Overview

The process for laboratory handling of a drug test result includes four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning denotes the sample's initial incorporation into the lab’s system. This requires ensuring that the sample was properly sealed and dispatched, issuing a random LAN (Laboratory Accessioning Number), and completing any supplementary data entry that the electronic chain of custody system may not cover.

Screening comprises a preliminary fast check for drug misuse. While Screening provides a cost-efficient method to dismiss drug usage in most samples, a positive screen must be confirmed for court admissibility. Any samples showing presumptive positivity in Screening necessitate additional confirmation.

If a sample tests presumptively positive during Screening, further hair is extracted from the primary specimen and prepped for Extraction. At this point, lower concentration drug extraction from hair takes place compared to other methodologies (e.g., urine or oral fluid), rendering hair drug testing distinctly challenging.

Verification of any positive screening outcomes occurs via GC/MS, GC/MS/MS, or LC/MS/MS. Every presumptive positive sample undergoes washing as needed prior to confirmation. The complete laboratory procedure from Accessioning through Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair program and the ISO/IEC 17025 accreditation standards.

In this special issue of Drug Testing and Analysis, we aim to showcase the remarkable manuscripts presented at this significant event, shedding light on the latest research and highlighting the future directions in this field.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
