For EtG, additional tests, and assessments beyond 10 panels, a 150mg sample is suggested. Utilizing a jeweler's scale to measure the sample is advised. If scalp hair isn't available, an equivalent quantity of body hair may be gathered. When discussing head hair, we are specifically referring to scalp hair. Body hair encompasses other hair types (such as facial and axillary hair).

Process Overview

The processing of a drug test result in the lab involves four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning incorporates the initial entry of a sample into a laboratory system. This process ensures that the sample was sealed and transported properly, assigns a unique LAN (Laboratory Accessioning Number), and concludes any remaining data entry not provided via an electronic chain of custody system.

Screening serves as an initial quick assessment for substance abuse. Though Screening is a cost-efficient approach to eliminate drug use in most samples, any positive screening must be validated to be accepted in legal proceedings. Samples that show a presumptive positive result during Screening necessitate further confirmation.

If a sample shows a presumptive positive in Screening, additional hair is drawn from the original sample and prepared for Extraction. During this phase, substances are extracted from hair at a reduced concentration compared to other methods (like urine or oral fluid), making hair drug testing the most challenging method to execute.

Confirming any positive screening result is done through GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positive results are cleaned before confirmation as needed. The entire laboratory process from Accessioning to Confirmation undergoes review as per the CAP (College of American Pathologists) Hair classification and the accreditation to ISO/IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
